I have been given this case to solve in an attempt to understand the topic of "patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with diagnosis and treatment 

 Patient came to OPD for routine check diagnosed with Huntingtons chorea 

Findings- Abnormal body posture,

Involuntary irregular movements in feet and face

Past History - Heart attack 10 yrs ago.

k/c/o - Diabetes mellitus since 1 year

No asthma, hypertension, thyroid, trauma.

Past Surgical History - stent surgery 10 yrs ago

Personal History- mixed diet, decreased appetite, inadequate sleep,normal bladder and bowel movements,no addiction

Family history - nil significance

General Examination -I have taken prior consent of patient.

Patient was examined in well lit room.

Patient is conscious , coherrent , co-operative, well oriented to time place and time.

No pallor , icterus , clubbing,

cyanosis , edema ,                    lymphadenopathy.

Pulse rate-68bpm

BP-120/80 mm hg
